Summary
When a school or department needs an archived cumulative student record from the Student Records Department office, the following process will be used.

- Any Salem-Keizer school or department staff with a business need to request a student cumulative folder
Definitions
CML file: Cumulative file containing records associated with a student's education, including documents, forms, etc.
Service Desk: The Salem-Keizer internal online system for submitting requests for support services.
Service Request: The internal ticket for requests for support services used within the Service Desk system.
Steps
- A school or department needs a student CML file from Student Records central office.
- A CML Request is submitted via a Service Request via the Service Desk:
- Access the Service Desk “Life Ring” on your desktop.
- Select the “Access Additional Services” button.
- In the Service Categories, select the “Technology” section, and select the button labeled “Student Cumulative Request."
- Completely fill out the request form providing as much data as possible:
- Requesting School
- Student First Name [required]
- Last Name [required]
- Middle Name [optional]
- SKPS Student Identification Number [required]
- Student Date of Birth (mm/dd/yyyy) [required]
- The Student Records central office team will:
- Locate the associated physical CML files.
- Package complete physical CML for delivery.
- Mail complete physical CML via In-District Mail Services.
- Add notification to customer service request and close request.
