Summary

Requirements for labeling a student cumulative (CML) folder. 

Intended Audience

Steps

  1. Use a label maker or printable label sheets to print and format labels for each student CML folder. Labels should be approximately 1 1/2" x 3 1/2" address label size (Dymo Label Printer, Avery Label 94205).
  2. Label folders in the following formatting:
  3. Label folders in the following convention:
    • Last Name, First Name Middle Name (or middle initial if too long to fit on top line of label) (example: Brown, Charlie Joe)
    • Salem-Keizer Student ID Number (example: 123456)
    • Date of Birth (in mm/dd/yyyy format) (example: 02/23/2012)
    • Grad Yr. (anticipated graduation year in yyyy format) (example: Grad Yr. 2030)
  4. All out-going (to another school in-district, or to the Student Records Department) folders are to be labeled in the above convention prior to sending.
  5. All 5th grade and 8th grade class folders should be labeled prior to transferring to the next level.
  6. For Legal Name Changes:
    1. Remake the label using the directions listed above using the new legal name and place over the top of the previous label.
    2. Make an additional label, following the above required formatting, with the previous legal name and place to the right of the main label with the following convention:
      • Previous Name: Last Name, First Name Middle Name (example: Previous Name: Doe, John Bob)
