This is a work instruction on how to fill out and send a student records transfer form when sending a student cumulative (CML) folder within the district.

Work Instruction

  1. Order Records Transfer form [INS-F013] from Market Direct Storefront #450210. (Digital Storefront)
  2. Fill out top portion of the form completely.
  3. Fill out bottom portion according to which records have been sent both physically and electronically by putting check marks next to the record type.
  4. Attach white top sheet by paper clipping or taping to the front of the student CML folder. Do not staple.
  5. The yellow sheet may be kept for senders records.
  6. Send completed sheet affixed to CML folder to the Student Records Department, or an in-district school via in-district office privacy mailer.
