This is a quick reference guide to requesting records when enrolling a new student.
When and how to request student records
Always check with the Student Records Department and within the "Person Search" in Synergy to see if a student has previously attended SKSP, as we may have a CML folder with their student history stored within our Student Records Department.
- Type "person search" into the Synergy search bar.

Search student by available information such as: name, SIS number, or date of birth and check the box next to "Student."

Select "Student Profile" button in the search results.

Review "Enrollment History" to determine where the student last attended.
Students who have previously attended a school within Salem-Keizer Public Schools
- Request records from their previous school within the district via email to the school's registrar if the enrollment history shows them as an "active" student. Physical cumulative (CML) folder to be transferred via inter-office mail.
- Request records from the Student Records department if the student shows as "inactive" in their enrollment. Physical cumulative (CML) folder to be transferred via inter-office mail.
Students who have never attended a school within Salem-Keizer Public Schools
- Request records from their previous school/district
- Create a new cumulative (CML) folder for the student (Student Cumulative (CML) Folder Labeling)
- Arrange records according to QAM TIS-W031 3.1
