This is a quick reference guide to requesting records when enrolling a new student.

When and how to request student records

Always check with the Student Records Department and within the "Person Search" in Synergy to see if a student has previously attended SKSP, as we may have a CML folder with their student history stored within our department.

Students who have previously attended a school within Salem-Keizer Public Schools

  • Request records from their previous school/district
  • Request records from the Student Records department to be transferred via inter-office mail
  • Shred any duplicate documentation, birth certificates, copies of parent/guardian driver's license, proof of residence, non-academic, and non-health related records
  • Arrange records according to QAM TIS-W031 3.1

Students who have never attended a school within Salem-Keizer Public Schools

  • Request records from their previous school/district
  • Create a new cumulative (CML) folder for the student
  • Arrange records according to QAM TIS-W031 3.1
